Save Time During Your Visit!

For our returning patients at Dental House, we are now offering the option to complete our forms all online to save time when you come in for your returning visit. To do this - download the form, fill-out your 6-month health history information, and upload it with your name and email below with your form attached.

If you would like to fill this out at our office, we ask that you please arrive at least 15 minutes prior to your appointment. Alternatively, you can download the form, fill-out your details, and bring them with you.

You will need to provide the following:

Contact Information
Emergency Contact Information
Insurance Information
6 Month Medical History
6 Month Dental History
A list of current medications you are taking
